Ed I found the problem. What's probably happening is that there is an error creating the deployment, like it can't retrieve the git revision or some permissions aren't in place. But this triggers a subsequent error.
I'll turn around a fix in a couple days, but in the meantime, can you change "CommandError" in the recipes.rb to "StandardError" and see what the result is? It probably won't submit the deployment, but it shouldn't break capistrano and you should see an indication of the real reason it's failing.
Can you follow-up to support@newrelic.com so I can keep you better informed?

Anyone who has this problem should contact support@newrelic.com. There are several possible causes for this.
It appears one cause of this problem is using SSL in the agent. If you have the use_ssl flag set to true, and you see mongrels that stop reporting, try setting it to false. We haven't isolated the exact cause for this because there has only been one reproducible incident, but in that case there were some plugins with suspicious patches to the Net:HTTP libraries.

Also, please change the log level to :debug in the newrelic.yml, and restart at least one mongrel. Include the newrelic_rpm.XXX.log file in your email.

Have you followed the instructions for modifying your deploy.rb adn Capfile?
Are there any errors in the capistrano output related to the deployments upload?
Can you try doing "cap newrelic:notice_deployment" and see if that was successful?
