Account lockouts occur when the same user from the same location is trying invalid passwords over and over again.
Turn off any 3rd party clients you might have (i.e. Tweetdeck, twitterrific, Tweetie, etc) on your phone or desktop.
Don't access twitter by any means for at least one hour.
Change your password on the site.
Change your password on all of the 3rd party desktop/phone clients you have to the current password.
If you're experiencing lockouts and you are absolutely certain that nothing from your location is trying to log into Twitter, let us know.

Log into Twitter.
Set up your phone on your Account Settings --> Devices page. Enter your phone number, then send an SMS to 40404 with the code word that the page displays to activate your phone.
Once that's complete:
If you're in the US, send a message to 40404 via SMS to send public tweets, or install an application on your cell phone capable of talking to Twitter.

There has been a large amount of discussion about the DoS Mitigation and how it impacts API users.

We know there is an issue with the opt-out link and we're patching that system to work correctly early next week.
Twitter will never 'bombard' anyone with messages to join. We only do it on user request and only send an invite once per user/invitee pair.
If she doesn't accept the request, her email will expire from the database in about a week.
(come to think of it, the fact that she got an 'oops' message means that her address was already expired.)
