Can you tell me the version of Firefox you are using? Firefox 2 is not supported so that could be an issue.

Also to make the feedback form go away, there should be an x in the corner that closes it.

If so there is a link in your email yes, but it will also show on your activity stream on "my home"

The original idea was to encourage people to leave a message, but we can allow that after the item is added.
